All applicable forms must be received by the office in order for your child(ren) to be properly registered.
Lead Risk Assessments are necessary for all students before they enter Kindergarten. Medical Forms are required for all new students and children entering grades K and 6. Dental Forms are required for all children entering grades K, 2, and 6. Eye Exam forms are required for new students to Illinois and children entering Kindergarten.
